Whats the formula for calculating max damage for Depurate Undead?

I've lost count how many times I've given this equation out - for the possible range of damages (neglecting the fact that some monsters are only partially undead, like Aet'Thol, the Ghoul and the Spirit of Ariat):
(Charisma * 3) + Level + 10 + (random number between 0 and 4)
For the maximum damage possible, it is:
(Charisma * 3) + Level + 14
(Charisma * 3) + Level + 10 + (random number between 0 and 4)
For the maximum damage possible, it is:
(Charisma * 3) + Level + 14
